POSITION SUMMARY
Nuxalk Health and Wellness is launching a Complex Care Housing (CCH) program for community members living with complex and concurrent mental health and substance use issues. Complex Care Housing aims to improve housing stability, improve health outcomes, improve community inclusion, reduce the use of acute care and emergency services, and reduce criminal justice system involvement.
Reporting to the CCH Coordinator, the CCH peer support positions will work with participants living in CCH. The ideal candidate has lived experience with mental health and/or substance use issues and currently is on their own wellness journey. They will have some knowledge of trauma-informed care and must be willing to work from a harm reduction perspective.
KEY ROLES AND RESPONSIBILITIES
- Works daily with clients to meet their care needs and support them with their wellness
- Implements multidisciplinary care plans, observes client’s behavior and symptoms and reports these to CCH care team
- Demonstrates skills to assist clients to develop physical, social, emotional and life skills.
- Involves client in activities of daily living, including maintaining personal hygiene and appearance, preparing food, safety, shopping, budgeting, laundering and cleaning, time management, organization and interpersonal communication, developing a social network.
- Participates in various client focused social and recreational activities which may include cultural activities, social groups, recreational activities, arts and crafts, therapeutic and educational groups and workshops.
- Acts as an advocate to assist clients in resolving conflicts with other clients
- Supports clients to attend appointments with Nuxalk Nation Health and Wellness staff as well as other health care professionals and service providers
- Ensures a safe environment for clients and staff by adhering to violence prevention, infection control, occupational health and safety standards.
- Maintains client records and provides input and feedback by participating in team care conferences and collaborates with the health care team to enhance client wellbeing.
- Reports incident/equipment problems or general safety concerns to CCH Coordinator.
- Attends and participates in staff development programs, team meetings, and other meetings as required.
- Supports the training of other staff as required
- Provides educational materials and supplies for harm reduction
QUALIFICATIONS
Qualifications include:
- Minimum grade 12 or GED completion
- Knowledge and experience working with Indigenous survivors of inter-generational colonial trauma,
- Knowledge and experience of mental health and/or substance use issues
- Ability to maintain confidentiality and strong boundaries within the cultural context of the Nuxalk Nation
- Experience with trauma-informed care and demonstrated ability to practice from a harm reduction approach would be an asset.
- A valid BC driver’s license and personal vehicle is required
- Food Safe Certificate
