POSITION SUMMARY:
The Complex Care Housing Social Worker serves as a key support for Nuxalk Nation community members with complex needs, including mental health challenges, substance use, chronic illness, disability, and housing instability. Grounded in cultural safety and respect for Nuxalk traditions and values, this role works directly with individuals and families to develop and implement holistic care and housing plans. The social worker collaborates closely with health providers, housing services, community leaders, and external agencies to ensure that members receive coordinated, wraparound supports. This position requires a trauma-informed, harm reduction and culturally responsive approach, with a deep understanding of the social determinants of health affecting Indigenous Peoples. The ideal candidate is committed to building trust, empowering clients, and advancing long-term wellness and housing stability in the Nuxalk community.
DUTIES AND RESPONSIBILITIES:
Client-Centered Support
- Conduct comprehensive assessments to identify housing, health, mental wellness, and social needs of individuals with complex care requirements.
- Develop and implement individualized care and housing plans in partnership with clients, their families, and community supports.
- Provide case management services, including referrals, service coordination, follow-up, and advocacy.
- Support clients navigating housing placements, tenancy issues, and transitions (e.g., hospital discharges, moving from temporary shelters to permanent housing). Support urgent referrals as needed
Cultural Safety and Community Engagement
- Deliver services in a culturally safe, respectful, and strengths-based manner rooted in Nuxalk values and traditions.
- Engage with Elders, cultural advisors, and family networks to integrate traditional knowledge and healing practices into care plans where appropriate.
Collaboration and Advocacy
- Work collaboratively with the Nuxalk Health and Wellness Team, Band Housing Department, Indigenous Services Canada, BC Housing, and local health authorities.
- Advocate on behalf of clients with service providers, legal systems, and housing authorities to address barriers and secure needed resources.
- Participate in interagency case conferences and planning meetings.
Monitoring, Documentation, and Reporting
- Maintain accurate and confidential case notes, reports, and client records in accordance with legal, ethical, and organizational standards.
- Monitor progress and adjust care plans in response to client needs and changing circumstances. Collects outcome data and statistics as required
Education/Training/Certification:
- Bachelor’s Degree in Social Work preferred. Candidates who are currently enrolled in a social work program are also encouraged to apply.
- Two (2) years’ recent experience working with marginalized clients with multiple barriers including mental health and substance use issues, homeless and complex trauma. An equivalent combination of education, training and experience may be considered.
- Local area travel may require the use of personal vehicle. Valid BC Drivers License.
- Indigenous ancestry and/or lived experience.
- Fluency in an Indigenous language preferred.
- Knowledge of local community resources and service providers
Salary Range:
$33-$37/hr depending on experience
