Annual Salary: $26.00 an hour with 3 month probationary period; $21,840.00
Tansi Friendship Centre Society is seeking a dedicated Social Prescriber to join our team in the Healthy Aging department. The Social Prescriber will be responsible for supporting aging adults in Chetwynd connect to non-medical services and activities to improve their health and wellbeing. They will meet with each client, listen to their needs, and connect them to local resources, social programs, and essential services like housing or food assistance. In this role, you will focus on building a supportive relationship with each person, helping them set goals that matter to them, and empowering them to take steps toward improved health and wellbeing.
Job Duties:
· Receive and manage referrals from healthcare providers and community partners
· Conduct follow-up and adjust support where needed
· Manage a caseload by understanding each client’s priorities and needs
· Promote social prescribing as a way to support people’s health and wellbeing
· Work on building connections with local healthcare and service providers to encourage client referrals
· Wellness Plan Development: Help clients create simple wellness plans that connect them with community groups and support services
· Facilitate warm connections to community resources and activities
· Meet clients in person, including some home visits if needed
· Work with healthcare providers as needed and refer clients back if they need additional services
· Collect relevant information to help track the client’s progress and outcomes
· Follow organizational policies, including those on confidentiality, lone work, and health and safety
· Participate in regular supervision meetings with your manager to discuss any challenges and get support
· Build connections with other community agencies by networking and attending events
· Join the Social Prescribing Program’s Community of Practice to learn and share best practices with colleagues across the region
· Map assets and identify service gaps
· Advocate for inclusive and accessible communities
· Prepare a quarterly report to the board of directors
· Track and manage client service data and conduct qualitative reporting
· Ability to manage a flexible schedule around clients
· Attend all staff meetings and the Annual General Meeting
· Maintain a high level of confidentiality in all interactions
· Maintain a professional image and demeanor with all employees, management and visitors at all times
· Assist with other duties as required
Qualifications:
· Certificate, diploma, or experience in social services, health, or a related field
· Strong understanding of community resources
· Previous experience working with older adults in a community or social services setting is an asset
· Knowledge of Indigenous culture, teachings and practices
· Demonstrate empathy, cultural humility, adaptability and a strong community lens
· Strong leadership skills that promote collaboration and learning
· Solid communication and interpersonal skills, both written and verbal
· Proficient with Microsoft Office (Word, Excel, Outlook, Power Point)
· A valid Emergency First Aid/CPR Certificate
· A clear Criminal Record Check for vulnerable persons
· Valid class 5 BC Driver’s License
Start Date: May 19, 2025
