Community Support and Outreach Worker

The Community Support and Outreach Worker provides direct, client-centered support to individuals facing barriers to social, economic, and community participation. This role focuses on outreach, advocacy, resource navigation, and community engagement to enhance client well-being and independence.

The position requires strong relationship-building skills, cultural sensitivity, and the ability to work collaboratively with community partners and internal teams.

Duties:

  • Conduct outreach to individuals experiencing barriers to services.
  • Provide one-on-one support, including needs assessments and goal planning.
  • Connect clients to community resources, services, and supports.
  • Advocate on behalf of clients with service providers and agencies.
  • Maintain accurate and confidential client documentation.
  • Facilitate workshops, information sessions, or support groups as needed.
  • Develop and maintain relationships with community partners.
  • Participate in case conferencing and team meetings.
  • Support data collection and reporting requirements.
  • Promote inclusive, trauma-informed, and culturally safe practices.

Working Conditions:

  • Combination of office-based and community-based work.
  • Will involve travel within the service area.
  • Occasional evening or weekend work.

Preferred Qualifications:

  • Experience in outreach or community-based service delivery.
  • Knowledge of local service networks.
  • Training in trauma-informed practice, harm reduction, or crisis intervention.
  • Knowledge of community resources and support systems.

Required:

  • Criminal Record Vulnerable Sector clearance is required if hired.
  • Diploma or degree in Social Work, Human Services, Community Development, or related field (or equivalent combination of education and experience).
  • Experience working with diverse populations facing complex barriers.
  • Strong communication, documentation, and interpersonal skills.
  • Ability to work independently and collaboratively.
  • Valid driver’s license and reliable transportation.
  • Must be reliable and dependable with excellent attendance; committed to reporting for all scheduled shifts.
