Community Connector – Social Prescribing Program

Position Type: Contract-based, permanent part-time (subject to program funding)

Office Hours: 10 AM – 2PM

Wage: $25–$31/hour depending on experience

Start Date: As soon as possible

Position Summary

The Community Connector helps bridge the gap between healthcare and social care by supporting seniors experiencing social isolation or non-medical challenges by connecting them to appropriate community services, programs, and supports.

Working as part of the Social Prescribing initiative supported by United Way, the Community Connector receives referrals from healthcare providers and community partners, works with participants to identify their goals, and connects them to local programs, services, and social opportunities that promote community engagement and overall wellbeing.

Key Responsibilities

•Receive and respond to referrals from healthcare providers and community partners.
•Conduct participant intake, needs assessments, and strengths-based goal setting.
•Connect participants to community resources such as social programs, food supports, transportation, housing resources, recreation, and volunteer opportunities.
•Provide follow-up support to ensure participants remain engaged and connected.
•Conduct outreach and build relationships with community organizations and service providers.
•Maintain an up-to-date directory of local services and supports.
•Develop ideas and initiatives to increase community awareness of the Social Prescribing program, including outreach and promotional activities.
•Support volunteer engagement and participation where appropriate to strengthen community connections.
•Assist with program promotion through community presentations, events, and partnerships.
•Coordinate with the Better at Home program and manage the Community Collaborative initiative as required by the funder.
•Collect program data, track participant outcomes, and prepare program reports as required by the funder.

•Provide updates and reports to the Executive Director of the North Cariboo Seniors’ Council.
•Participate in Social Prescribing Community of Practice sessions, webinars, meetings, and provincial summits organized through United Way British Columbia.
•Maintain confidentiality and follow organizational policies and privacy standards.

 

Qualifications

•Bachelor’s degree or experience in social services, community development, health promotion, or a related field.
•Strong knowledge of seniors’ issues and challenges related to healthy aging.
•Minimum 2 years experience working with seniors or vulnerable populations from different cultures and socio-economic backgrounds.
•Knowledge of community resources in the North Cariboo region.
•Strong communication, relationship-building, and organizational skills.
•Ability to work independently and collaboratively with community partners.
•Valid driver’s licence, driver’s abstract, insurance and access to reliable transportation may be required.
•Criminal record check (vulnerable sector) required.
