Indigenous Patient Navigator
We are seeking an Indigenous Patient Navigator to support community members in accessing safe, respectful, and coordinated healthcare services within our 5 communities and through referral systems.
This role serves as a trusted liaison between patients, families, healthcare providers, and external hospitals. The Indigenous Patient Navigator assists with medical travel, referrals, follow-up care, and advocacy, while ensuring services are culturally grounded and aligned with community values.
The position supports reconciliation efforts consistent with the Calls to Action of the Truth and Reconciliation Commission of Canada and the principles of the United Nations Declaration on the Rights of Indigenous Peoples.
Key Responsibilities:
- Support patients in understanding treatment plans and referrals
- Coordinate medical travel and follow-up appointments
- Advocate for culturally safe, trauma-informed care
- Connect individuals with local and regional resources
- Build strong relationships with Elders, families, and care teams
Qualifications:
- Strong connection to Indigenous communities (local applicants encouraged)
- Experience in healthcare, social services, or community-based work
- Understanding of systemic barriers impacting Indigenous health
- Ability to work independently in a remote setting
