  • Proven cash handling experience.
  • Demonstrated skating experience.
  • Demonstrated ability to problem solve and multi-task in a fast-paced environment.
  • Compliance with and approval of Security/Reliability Clearance and Criminal Record Search.

South Peace Primary Care Network Coordinator

PRIMARY CARE NETWORK COORDINATOR
In British Columbia’s beautiful South Peace Region a strong collaborative partnership approach has led to the co-creation of the South Peace Primary Care Network (PCN). Anchored by a locally Co-Chaired Primary Care Network Steering Committee that is dedicated to providing quality team-based primary care to patients and families, the South Peace is seeking a Primary Care Network Coordinator.

POSITION SUMMARY
Reporting to the PCN Manager or delegate, the PCN Coordinator assists with project implementation, administrative coordination, physician engagement, and stakeholder communication. This role collaborates with physicians, health authorities, Indigenous communities, and other partners to improve healthcare access in the South Peace Region.

The Coordinator works with the South Peace Division of Family Practice, Northern Health Authority staff, Patient Medical Homes (PMHs) primary care providers (Family Physicians and Nurse Practitioners), local Indigenous communities and Nations, First Nations Health Authority, the Regional Health Authority, and community agencies/organizations in development of the PCN.

RESPONSIBILITIES
Support program leaders with PCN projects and initiatives.
Assist in developing and implementing project work plans, identifying strengths and areas for improvement.
Coordinate and strengthen member and stakeholder engagement, building relationships with physicians, project partners, and other key stakeholders.
Organize and facilitate meetings, including agenda setting, distributing materials, and tracking action items.
Provide regular reports on PCN activities and outcomes, keeping project teams, leadership, Division members, and funding partners informed.
Develop resources such as manuals, communication briefs, emails, and website updates for both members and the public.
Collaborate on evaluation and quality improvement initiatives, gathering and analyzing data as needed.
Work with colleagues to improve operational processes and procedures.
Navigate diverse personalities and social dynamics, adapting communication styles to different emotional and cultural contexts.
Understand team and committee dynamics, fostering collaboration and maintaining a strong, connected environment.
Proficient in Microsoft 365, Canva, social media management, and familiar with project management tools and communication platforms such as Zoom.

Duties:

Summary Statement

Esk’etemc is seeking a highly motivated and innovative individual to work as the Community Health Nurse (CHN), who will report to the Health Director.

This position is responsible for the provision of a comprehensive range of core nursing functions and services in Esk’etemc, in program areas of community heath, through promotion and maintenance of the health of individuals, family groups, and community members, that value the principles of primary health care and focus on promoting health, preventing disease and injury, protecting population health as well as, when required, a focus on curative, urgent and emergent care, rehabilitation and supportive or palliative care.

The CHN will work a standard 35-hour work week (Monday – Friday); but, must be flexible in scheduling work hours to best accommodated clients, activities, and other functions as required.

Duties and Responsibilities

  • Understands and respects First Nations cultural principles and protocols and is able to apply this understanding in work situations.
  • Collects, assesses, analyzes and applies information from various data sources to make evidence informed decisions for nursing services, program planning, development and priority setting with individuals, families, groups, and community members and interprets information for professional and community audiences.
  • Involve individuals, families, groups, and community members as active partners to take action to address health inequities and foster a self management care approach for chronic conditions.
  • Leads the emergency medical, clinical, or community response team in the absence of a doctor or a nurse in charge; assigns responsibilities, monitors activities, provides support and guidance in the rapid resolution of issues, and provides overall coordination.
  • Collaborate with members of the health team to plan, implement, co-ordinate and evaluate patient care in consultation with patients and their families.
  • Administer medications and treatments as prescribed by a physician or according to established policies and protocols.
  • Provide health education and registered nursing through home visits, manage complex home care cases, participate in community needs assessment and program development, conduct disease screening and deliver immunization programs.
  • May give injections and medications, dress wounds, and assist other registered nurses.
  • Provide health education to community members regarding protection against communicable disease.
  • Arranging and participating in joint home visits to families with other professionals.

Qualifications

  • Bachelor’s degree in nursing from a recognized University.
  • Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
  • One (1) year of nursing experience, specifically public health
  • Requires a valid Class 5 BC Driver’s License

Knowledge Skills and Abilities

  • Experience as a nurse in settings such as acute care and emergency care.
  • Knowledge of health status of populations, inequities in health, the determinants of health and illness, principals of primary care, strategies for health promotion, disease and injury prevention, health protection, curative, urgent and emergent care, rehabilitation and supportive or palliative care.

The Letwilc Ren Semec Centre (Recovery Centre) is seeking a very positive and energetic person to fill the position of Life Skills Worker. The successful candidate will be under the direction of Recovery Support Manager.  The Life Skills Worker position is part-time position and is responsible for the delivery of client focused, culturally relevant and trauma informed support services and programs that address resident needs in an appropriate and effective manner.

The Life Skills Worker will work a Monday to Friday 8 hrs, 12:00am to 8:30 am @ 8 hrs a shift for 40 hrs. a week total 80 biweekly but must be flexible in scheduling work hours to best accommodate clients, activities, public events and other functions as required.

  • Specific Duties
    Orient clients to recovery home, and provide background on Recovery Centre and the community.
    Assist residents to establish, maintain, and enforce appropriate standards of behavior and attitude
  • according to established house guidelines.
  • Supervise and support residents in the performance of work, chore or volunteer assignments.
  • Motivate and facilitate adjustment to change and help residents define personal behavioral objectives.
  • Provide resident support and guidance as required, includes maintenance of resident safety and to take remedial action if and as required.
  • Serve as key worker to the residents, ensuring consistent staff contact, case planning and communication for those individuals.
  • Support the residents in following care plans developed, maintain independence in the community, and/or access community options or referrals. Continuously evaluate, the outcomes of existing life skills services and current needs of the resident group.
  • Research emerging best practices and resources in life skills and pre-employment support.
  • Develop life skills and pre-employment skills workshops relevant to participants’ needs and capacity.

Education

  • Grade 12 or equivalent
  • Driver’s License would be an asset

Professional Abilities

  • Minimum two years’ experience in substance misuse and mental health programs and experience working with Indigenous clients an asset. First Aid, emergency transport endorsement and C.P.R. Certification.
  • Good communication and interpersonal skills.
  • Ability to effectively report information to employees and management.
  • Effective problem-solving skills, including conflict resolution.
  • A committed team player, detail oriented, resourceful and an effective communicator.
  • Professional Certification from Life Skills training program and two years’ work experience in recovery home.

  • Assist with client transportation and adult day program.
  • Assist with housekeeping and home management services in accordance with Health & Safety Standards. The client’s care plan, including house cleaning, laundry, ironing, meal planning and preparation, safety and security checks, shopping, light snow removal or sidewalk clearing related to the safety of clients and related services.
  • Provides short-term (hourly) respite services to allow family care givers some personal time.
  • Under the supervision of the Home & Community Care Supervisor provides information and education to clients and family members regarding home management and housekeeping techniques that will assist the client and family with self-care.
  • Provides support to clients and accompanies them to social, cultural or recreational activities where appropriate.
  • Assists clients in keeping appointments with various health services.
  • Monitors and observes changes in the health status and observes clients behaviours; regarding, their living situation and reports these to the Home & Community Care Supervisor.

Education

  • Health Care Assistant Certificate and supervised practical training are required.
  • Completion of specialized courses such as CPR (cardiopulmonary resuscitation), first aid and food handling/sterile processing.

  • Experience delivering health or social services in a First Nations setting
  • Knowledge of current First Nations health issues
  • Knowledge of the common acute, chronic and emergent health conditions
  • Ability to adapt health services to meet local community needs and to ensure safety and sensitivity.
  • Ability to communicate orally and in writing.
  • Extensive experience in entering client health information into database for monitoring.
  • Valid Class 5 Driver’s Licence
