Office Hours: 10 AM – 2PM

Wage: $25–$31/hour depending on experience

Position Summary

The Community Connector helps bridge the gap between healthcare and social care by supporting seniors experiencing social isolation or non-medical challenges by connecting them to appropriate community services, programs, and supports.

Working as part of the Social Prescribing initiative supported by United Way, the Community Connector receives referrals from healthcare providers and community partners, works with participants to identify their goals, and connects them to local programs, services, and social opportunities that promote community engagement and overall wellbeing.

Key Responsibilities

•Receive and respond to referrals from healthcare providers and community partners.
•Conduct participant intake, needs assessments, and strengths-based goal setting.
•Connect participants to community resources such as social programs, food supports, transportation, housing resources, recreation, and volunteer opportunities.
•Provide follow-up support to ensure participants remain engaged and connected.
•Conduct outreach and build relationships with community organizations and service providers.
•Maintain an up-to-date directory of local services and supports.
•Develop ideas and initiatives to increase community awareness of the Social Prescribing program, including outreach and promotional activities.
•Support volunteer engagement and participation where appropriate to strengthen community connections.
•Assist with program promotion through community presentations, events, and partnerships.
•Coordinate with the Better at Home program and manage the Community Collaborative initiative as required by the funder.
•Collect program data, track participant outcomes, and prepare program reports as required by the funder.

•Provide updates and reports to the Executive Director of the North Cariboo Seniors’ Council.
•Participate in Social Prescribing Community of Practice sessions, webinars, meetings, and provincial summits organized through United Way British Columbia.
•Maintain confidentiality and follow organizational policies and privacy standards.

Qualifications

•Bachelor’s degree or experience in social services, community development, health promotion, or a related field.
•Strong knowledge of seniors’ issues and challenges related to healthy aging.
•Minimum 2 years experience working with seniors or vulnerable populations from different cultures and socio-economic backgrounds.
•Knowledge of community resources in the North Cariboo region.
•Strong communication, relationship-building, and organizational skills.
•Ability to work independently and collaboratively with community partners.
•Valid driver’s licence, driver’s abstract, insurance and access to reliable transportation may be required.
•Criminal record check (vulnerable sector) required.

Full-time Summer Position with potential for extension.

Dedicated to advancing sport participation and excellence in the north, Engage Sport North provides high quality sport participation and development services and programming to athletes, coaches, officials and sport leaders. In addition, Engage Sport North offers introduction to sport opportunities and physical literacy skill development to children and adults. Engage Sport North works with people of all ages and abilities from those new to sport to experienced athletes and coaches on a high-performance pathway.

Key Responsibilities and Duties

As an Engage Sport North Leader your duties will include:

• Lead Sport Discovery Programming such as Community Multi-Sport programs, Active Star, Try-it Programs and day camps.
• Providing a fun environment and encouraging all children to participate in all activities and teaching physical literacy through sports and games;
• Effectively problem solve and communication with all staff, participants, and caregivers;
• Being always aware of the surroundings and the location of all participants;
• Maintaining a safe environment and protecting the health and safety of self, participants, and staff;

Education, Experience and other assets:

• Grade 10 education or higher
• Front-line experience working with children and youth;
• Experience coaching or participation in recreation and/or high-performance sports. Coach training will be provided.
• Outgoing, energetic, self-starter who will have fun with children while leading sport activities.

Skills and Attributes:

• Organizational skills to plan daily activities & carry them through effectively and while paying attention to detail;
• Effective verbal and written communication.
• Good Customer service with the ability to communicate with caregivers in a professional manner.
• Work proactively with minimal supervision;
• Ability to work in a team environment;
• Ability to think critically and act logically, evaluating situations and generating steps to achieve success.

Requirements:

• Eligible to work in Canada;
• Applicant must be able to provide a criminal record check for the Vulnerable Sector;
• First Aid and CPR training is an asset.
• Have reliable transportation to and from recreation facilities in the Quesnel area
• Availability to work 8 weeks consecutively throughout the summer
