Job Summary:
The LPN in Home and Community Care (HCC) works collaboratively with the RN or Nurse Practitioner and other interdisciplinary team members. The scope of practice for LPNs dictates nursing care that is within their knowledge and ability. They will be required to practice with their level of competence. They are also required to seek direction and guidance from other health care professionals when aspects of the care required are beyond their individual competence. LPNs can provide care independently to clients with predictable outcomes, the LPN will work in collaboration with the RN or NP, and in some cases the RN or NP may assume the care of the client.
Assessment:
The RN or NP will complete the admission assessment. The LPN performs ongoing assessment to identify the status of the client’s actual or potential limitations and strengths to determine if unique needs and preferences are being met.
Implementation:
The LPN implements the following skills in accordance with their standards of practice, competencies and according to the plan of care.
Evaluation:
- Monitors and recognizes changes in client status and response to interventions
- Communicates the changes and collaborates in modifying the care plan in order to achieve better outcomes
- Measures and supports client’s progress in achieving goals identified in the care plan
Education:
- Licensed Practical Nurse (LPN) – Current CLPNBC registration
- Preference given to someone with one year minimum experience working in a community setting, and preferably a First Nation setting
Preferred Knowledge and Skills:
- Excellent interpersonal & communication skills to deal with case sensitive clients of various age groups
- Knowledge of diabetes testing, diagnosis and management techniques
- Knowledge of First Nation adult learning styles and innovative teaching, asset to have knowledge of the Bonaparte Indian Band
- Strong organizational, record-keeping and data management skills
- Strong computer skills
- Reliable and dependable
- Excellent communication and interpersonal skills
- Ability to work collaboratively with and independently in the community
Mandatory Requirements:
- Valid credentials
- Valid Class 5 Driver’s License
- Reliable transportation including insurance
- Must clear criminal record check
- Level C First Aid
- Ability to meet the physical demands of the Home Care Program
